*Bring the following items on the above dates. 1) Birth Certificate, 2) Social Security Card, 3) FL Physical Address

Eligibility: 4 year old by September 1, 2008, and Florida Resident

Procedure: 1) A voucher will be issued, 2) Parent will carry voucher to a provider of their choice, 3) Parent will complete registration package including provide immunization record, and physical for the chosen site.
